MTC completion check sheet
Check that the following information is correct:
- Are you authorised to complete an MTC?
- Establishment number and slaughter date recorded.
- Product description/ number of cartons in numbers and words in brackets eg 6 (six).
- ID marks for specific markets entered.
- Production establishment production date and date/s packed.
- Line drawn to the end of the product list so nothing else can be added to the certificate.
- Market eligibility marks/statements.
- Halal service provider entered if applicable.
- Restriction stamp for Ecoli -0157:H7 if required.
- Restriction stamps for product not eligible for USA eg all short loins over 30 months if required.
- The customer and their establishment number.
- Establishment details ie name, address, phone number, establishment number.
- Approximate departure time, approximate travel time.
- Transport company name/ trailer number/seal number/s.
- Name of person competing MTC, signature and date MTC. Was the load subject to detailed inspection yes/no.
- If the establishment is EU listed the AQIS endorsement section needs to be completed by the AQIS OPS.
- MTC transfer docket number must be written on the MTC.
- White/pink copy to customer and the green/yellow copy kept by the establishment.
- All mistakes must be crossed out and initialled.
